Primary care, hospital, and specialty offices can send a patient to Summit Kidney and Hypertension Care in one step. Complete the referral form, attach the records listed below, and fax it to our office. We contact the patient to schedule.

How a referral works
Complete the referral form with your office contact details, the patient's name, date of birth, phone number, and insurance, and the reason for the referral.
Attach the records on the checklist so Dr. Baig can review the referral before the visit.
Fax the form and records to our office. Mark the referral urgent or routine on the form.
We contact the patient directly to schedule. Your office is welcome to call with questions at any point.
Urgent referrals, such as rapidly declining kidney function or follow-up after a hospital stay, are reviewed within 24 to 48 hours. Routine referrals are scheduled at the next available appointment.
Reasons to refer
Dr. Baig accepts referrals for any of the following. If you are unsure whether a patient needs nephrology care, call the office and ask.
- Chronic kidney disease
Any stage, including a new or worsening decline in eGFR.
- Proteinuria or microalbuminuria
Protein in the urine on a dipstick, spot ratio, or 24-hour collection.
- Uncontrolled or resistant hypertension
Blood pressure above goal on three or more medications.
- Hematuria
Gross or microscopic blood in the urine.
- Electrolyte abnormalities
Potassium, sodium, calcium, or magnesium outside the normal range.
- Kidney stones
Nephrolithiasis, especially recurrent stones needing a metabolic workup.
- Acute kidney injury or elevated creatinine
Including follow-up after a hospital stay.
- Other consultation requests
Any question where a nephrologist's opinion would help.
What to send with the referral
These records let Dr. Baig review the referral before the visit and set the right urgency. Send what you have; missing items will not hold up the referral.
- Patient demographics or face sheet
- Recent labs: eGFR, serum creatinine, and basic metabolic panel
- Front and back of the insurance card
- Urinalysis or urine protein-to-creatinine ratio
- Clinical notes from the last two visits
- Renal ultrasound or imaging reports, if available
Are you a patient?
You do not need this form. Many insurance plans require a referral from your primary care provider before a specialist visit, so check with your plan, then book online or call the office and we will help.
